 Application Form
Completed and signed Application Form (IMM5257) and Family Information Form (IMM5645)
 Passport
Valid for the duration of the anticipated trip with at least one blank page
 Photos
Two identical recent passport photographs against a light background

 Minor Children
Children younger that 18 years not traveling with both parents should submit a notarized parental consent letter signed by a Notary Public
 Flight Itinerary
Itinerary on a letterhead showing dates of entry and departure and flight information

 Employment Letter
Stating the duration of leave, date employment was commenced and a confirmation of your position and if self-employed, provide a letter on a letterhead as well as the registration certificates to proof ownership
 Accommodation
Confirmed hotel booking(s) i.e. Reservations or

 Sufficient funds
Three months bank account statements, term deposit statements or tax papers as proof of sufficient funds.
